子宫纵隔切除患者的IVF/ICSI-ET临床结局分析

Clinical Outcomes of IVF/ICSI-ET in Infertile Women after Transcervical Resection of Septum

摘要： 目的：探讨子宫纵隔切除术（transcervical resection of septum，TCRS）后不孕患者的体外受精/胞浆内单精子注射-胚胎移植（IVF/ICSI-ET）临床结局。方法：回顾性分析2010年1月—2014年12月在首都医科大学附属北京妇产医院生殖医学中心行IVF/ICSI助孕并有TCRS史的35例患者的临床资料（观察组），随机抽取同期子宫形态正常的不孕患者70例作为对照组，比较2组的基本情况及妊娠结局。结果：观察组既往流产次数高于对照组，差异有统计学意义（Z=6.224，P＜0.001）。2组患者年龄、不孕时间、获卵数、受精率、优质胚胎率、临床妊娠率、异位妊娠率、活产率和早产率差异均无统计学意义（均P＞0.05）。结论：TCRS能够纠正宫腔形态异常，使患者获得良好的IVF助孕结局。

Abstract: Objective: To investigate the clinical outcomes of in vitro fertilization or intracytoplasmic sperm injection-embryo transfer （IVF/ICSI-ET） in patients after transcervical resection of septum （ＴＣＲＳ）. Methods：Clinical data of 35 infertile women with a history of TCRS and underwent IVF/ICSI-ET from January 2010 to December 2014 were analyzed retrospectively. Seventy patients with a normal uterus and received IVF/ICSI-ET homeochronously were randomly selected as control. The IVF/ICSI-ET outcomes of both groups were compared. Results：Patients with a history of TCRS had a significantly higher miscarriage rate than the control group （Z=6.224，P＜0.001）. There were no significant differences in the patients′ age, infertility duration, number of oocytes retrieved, rates of fertilization and high quality embryo, rates of clinical pregnancy and ectopic pregnancy, rates of live birth and preterm delivery between two groups （Ｐ＞０．０５）. Conclusions：After abnormal morphology of the uterine cavity were rectified by TCRS, infertile patients with uterus septum could achieve satisfactory IVF outcomes.
