    A Case Report of Prenatal Diagnosis of 69, XXX Homozygous Triploid

    A case of 69, XXX homozygous triploid was reported. This is a 24-year-old, 21-week-pregnant woman with adverse pregnancy and childbirth history. The serum prenatal screening showed the high risk of 18-trisomy syndrome. The four-dimensional color ultrasound suggested some abnormality, and the non-invasive prenatal testing (NIPT) suggested the high risk of sex chromosome aneuploidy. After that, the in-depth genetic testing and analysis by the short tandem repeat (STR) fragment analysis and fetal chromosome karyotype analysis were done with the informed consent. In this case, the fetal chromosome karyotype was 69, XXX, and the STR fragment analysis showed fetal triploid. The possible cause of this case was double female fertilization, since no maternal contamination was found in amniotic fluid samples. Therefore, this fetal case was considered as the homozygous triploid. The pregnant woman chose to terminate the pregnancy after genetic counseling. In conclusion, the prenatal diagnosis combining with STR fragment analysis can not only eliminate the influence of maternal contamination, but also find chromosome number abnormalities.

    The 16p11.2 syndrome is caused by the microdeletion or microduplication of chromosome 16p11.2 region, which is characterized by extensive heterogeneity and incomplete penetrance. The phenotype of 16p11.2 microdeletion is characterized by developmental retardation, intellectual disability, autism spectrum disorder (ASD), and morbid obesity, while the phenotype of 16p11.2 microduplication is characterized by ASD, schizophrenia, intellectual disability and microcephaly. Two core susceptible regions of 16p11.2 are a distal breakpoints (BP) at 28.8-29.0 Mb (BP2-BP3) of 220 kb and a proximal BP at 29.6-30.2 Mb (BP4-BP5) interval of 593 kb. Recurrent rearrangements of nonallelic homologous recombination between highly similar duplicated sequences lead to the deletion or duplication of 16p11.2. With the wide application of chromosome microarray analysis as a prenatal diagnostic technique, chromosomal microdeletions and microduplications are increasingly recognized. However, there is currently no effective therapeutic method for 16p11.2 syndrome. Therefore, a detailed understanding of the clinical manifestations, mechanism and candidate genes of 16p11.2 syndrome may provide a basis for genetic counseling.

    Function of Anti-Mullerian Hormone in Ovary-related Diseases

    Anti-Mullerian hormone (AMH), a disulfide-linked dimer of two identical subunits of glycoprotein, is secreted by immature testicular Sertoli cell and ovarian granulosa cells, belonging to the transforming growth factor β super-family. One of main physiological functions is regulating the germ cell development. AMH participates in regulating follicular development and growth. AMH has been used to assess ovarian reserve and to predict menopause age. It can be used to detect early ovarian disease and to assess progression and prognosis of disease. In the area of assisted reproductive technology (ART), the application of AMH gets more and more recognition. With the development of analysis technics, AMH test gets more simple and accurate. Compared with other biological indicators, AMH is relatively stable in the different day of menstrual cycle. Now, AMH, as well as its application in ART, has become a hot-topic in recent years. In this paper, we summarized the function of AMH and its clinical application in ovary-related diseases.

    Jinfeng Pill Combined with Recombinant Human Growth Hormone Improved Endometrial Receptivity in Patients with Thin-Endometrial Infertility

    Objective: To observe the clinical effect of Jinfeng Pill combined with recombinant human growth hormone (rhGH) on endometrial receptivity in patients with thin endometrium and infertility. Methods: 80 infertile patients with thin endometrium in the department of reproductive medicine of our hospital from December 2018 to December 2019 were included. The patients with thin endometrial infertility were treated with Jinfeng Wan and recombinant human growth hormone (40 cases) in the observation group, and only routinely treated (40 cases) in the control group. Endometrial thickness, endometrial typing, subendometrial blood flow, clinical pregnancy rate, and abortion rate were compared between the two groups. Results: There were no significant differences in the age of egg retrieval, infertility, body mass index (BMI), basic follicle stimulating hormone (FSH), number of embryo transfers, embryo transfer, and the endometrial type, subendometrial blood flow type, endometrial thickness before treatment between the two groups (P>0.05). The endometrial thickness of the observation group was higher than that of the control group on the day of transplantation, and the pregnancy rates of type A and type B endometrial patients were higher than that of type C endometrial patients. The pregnancy rate of patients with type II and type III endometrial blood flow was higher than that of patients with type I endometrial blood flow (P<0.05). The clinical pregnancy rate was higher and the miscarriage rate lower in the observation group when compared with the control group (P<0.05). Conclusions: Jinfeng Pill combined with rhGH can improve endometrial receptivity in patients with thin-endometrial infertility, thereby increase clinical pregnancy rate and improve pregnancy outcomes.

    Epimedium is a commonly used traditional Chinese medicine. Icariin as a flavonoid extracted from Epimedium is the main active substance in Epimedium. It is found that Icariin has many functions, such as promoting the formation and activation of osteoblasts, regulating immunity, anti-aging, anti-inflammatory and so on. Clinically, Epimedium(Icariin) is used to treat many diseases of reproductive system, bone and joint, respiratory system, nervous system, cardiovascular system, immune system and other. This paper summarizes the effects and mechanisms of Epimedium(Icariin) on male reproductive system. Epimedium(Icariin) plays an androgen-like effect by itself, and promotes the synthesis and secretion of testicular testosterone. Icariin can promote the production of sperm, increase sperm density, improve sperm motility, and slow down reproductive decline by improving the microenvironment of spermatogenesis and antioxidation. In addition, Epimedium(Icariin) is also used to treat the erectile dysfunction and premature ejaculation by promoting penile erection.

    Tight junction is a fundamental junctional complex of the adjacent epithelial cells,with multiple functions including the barrier effect regulating intercellular permeability and maintaining cellular polarity, which was consisted of several protein families including transmembrane proteins (such as occluding, claudin) and peripheral proteins (such as ZO protein). The structure and function of tight junction can be changed by many physio-/pathological factors such as anoxia, hormonal, inflammatory factors and estrogen. Tight junction has already been detected in human uterine endometrium and placenta. Factors of tight junction expressed in those tissues and organs. Abnormality of tight junction may be involved in the pregnancy complications.

    Objective: To analyze the clinical and pathological characteristics of gastric-type adenocarcinoma (GAS), in order to diagnose and treat the disease early and improve the prognosis. Methods: The diagnosis, treatment, follow-up, pathological morphology, immunohistochemical characteristics and other data of 9 cases of cervical GAS in Tianjin Central Hospital of Gynecology Obstetrics from July 2013 to July 2017 were retrospectively analyzed, and relevant literatures were reviewed. Results: The median age of 9 patients was 44 years old. 6 patients were diagnosed in pre-operation, 2 in intra-operation and 1 was in post-operation. There were 3 cases of long-term abnormal vaginal discharge, 2 cases of intrauterine fluid and 5 cases of ovarian mass. The mean overall time to survival was 104.3 months (95%CI: 66.6-142.1 months). The survival rates at 12 months, 24 months and 36 months were 88.9%, 77.8% and 66.7% respectively. The cervical GAS mainly invaded towards the uterine body. The pathological microscope showed little difference from the normal gland. Immunohistochemical analysis of HIK1083, Mucin 6 (MUC6), cytokeratin7 (CK7), CDX2, villin, paired box gene-8 (PAX-8), CA-125, CA-19-9, P16 and carcinoembryonic antigen (CEA) showed high positive rates, while the positive rates of CK20, PAX-2 and P53 were low. Conclusions: Cervical GAS is rare and easy to be misdiagnosed. However, through the multidisciplinary combination of clinical, imaging and pathology, there is an opportunity to achieve the early diagnosis and treatment of GAS and improve the survival expectation.

    Prenatal brain damage is the results of coaction of many factors,which can induce pathological action of central nervous system and neuropathological changes. Those infants and children may have deficiencies or disorders of learning and memory,behavior and movement audio-visual ability and
    congnition after birth. The mechanism of perinatal brain damage on neurological development after birth is unidentified yet. It could be related to pathophysiologic changes of cerebral tissue,unbalance of receptors and receptor-related channels of nervous system and malajustment of cerebral cytokines:decrease of cerebral cell,injury of cerebral white matter,decrease of the weight of hippocampal area and overload of endocellular calcium, which are the main pathophysilolgic changes of cerebral tissue; the expression of NMDA receptor,dopamine receptor and glucocorticoid receptor may change; cerebral cytokines and BDNF may have unbalanced environment, and abnormal elevated levels of NSE at birth as well as progressive increase of PS-100 could be found in the newborns during the first week of life.

    The Application of PAPP鄄A on Down’s Syndrome Screening at the Early Pregnancy

    Down’s Syndrome, which also named Trisomy 21,is the first discovered and the most common autosomal disease in human being. It is with serious harm and high incidence of abiogenesis. It has an important significancefor prenatal screening. Pregnancy assoociated plasmaprotein A(PAPP-A) is an effective maternal serum markers of Down’s syndrome screening at the early pregnancy, and the effect of the single detection is equal to or better than the other serum markers. The best combination of the double-detection is PAPP-A+β-hCG. The multiple-detection with NT improved obviously, and PAPP-A is the most promising index of Down’s syndrome screening at the early pregnancy. At present, the main screening index is only for the single-pregnancy, and the screening index of twin pregnancy is lack of a unified, common value standards which need to be improved.

    Objective: Orphan nuclear receptor NR4A1, a member of the nuclear receptor superfamily,is widely expressed in different cell types and mediates diverse biological processes. In the present study,our main purpose was to explore the function of NR4A1 in the transcript regulation of steroidogenic enzyme genes responsible for ovarian theca cell steroidogenesis. Methods: We constructed recombinant adenovirus AdCMV?-NR4A1 and AdH1-NR4A1 to enhance or knockdown the expression of NR4A1 in theca cells,respectively. The expression patterns of Cyp11a1, Cyp17a1 and Hsd3b2 were subsequently analyzed by real?-time RT-PCR. Moreover,concentrations of testosterone in the spent medium were measured by radioimmunoassay. Results:Our results show that overexpression of NR4A1 in theca cells stimulates the expression of Star, Cyp11a1, Cyp17a1 and Hsd3b2, leading to increased testosterone production. Conversely,knockdown of the endogenous NR4A1 exhibits a significant decrease in Cyp11a1, Cyp17a1 and Hsd3b2 expression and testosterone production. Forskolin(FSK), an activator of cAMP/PKA pathway,was applied to the cultured follicles. FSK rapidly increases the Nr4a1 mRNA levels followed by an increase in Cyp11a1, Cyp17a1 and Hsd3b2. Conclusions:Collectively, our results outline a previously unrecognized role for NR4A1 in the transcriptional regulation of Cyp11a1, Cyp17a1 and Hsd3b2 in ovarian theca cells. Modulation of these steroidogenic enzymes by NR4A1 could influence the capacity of the ovarian theca cells to produce androgen. This study provide basis for us to further explore the mechanism of NR4A1 on the pathophysiology development of polycystic ovary syndrome hyperandrogenism at molecular level.

    Thin endometrium can lead to the decreased rates of clinical pregnancy and live birth. At present, there is no optimal plan to improve endometrial thickness in patients with thin endometrium. As an auxiliary method for emerging endometrial preparation, platelet-rich plasma (PRP) perfusion therapy may have a complex mechanism in the treatment of thin endometrium. The platelet concentration in PRP is higher than that in whole blood. The basic principle and therapeutic potential of high concentration platelets are based on their ability to provide superphysiological quantities of essential growth factors for regenerative stimulation and to promote tissue repair with low healing potential. Based on the research progress of PRP, this paper focuses on the growth factors in PRP to promote cell proliferation, migration, and to regulate the proliferation and apoptosis of endometrial cells through NF-κB signaling pathway. The discussion will provide a reference for further study of the molecular basis and exact mechanism of PRP therapy.

    The very low cut-off value for sperm morphology of 4%morphologically normal spermatozoa,as proposed in the new edition of the World Health Organization (WHO)manual on semen analysis,is in agreement with recently published values and reflects the trend of a decline in reported mean values for normal
    sperm morphology. The reduced value for morphologically normal spermatozoa over the years may be due to several factors. The first is the introduction of strict criteria for the evaluation of sperm morphology. Other reasons may include the introduction of additional criteria for sperm morphology abnormalities and the suggested decrease in semen parameters because of increasing negative environmental influences. Although on its own the newly proposed very low normal value may not provide the strong predictive value for a males’ fertility potential,as originally reported for sperm morphology evaluated according to strict criteria,a good predictive value can still be obtained if the holistic,strict approach for sperm morphology evaluation is
    followed together with additional sperm morphology parameters now available,because certain morphology patterns and sperm abnormalities are now known to be of strong prognostic value. In addition,better international standardization of the technical methodology, consensus on the interpretation of sperm morphology evaluation criteria and standardized international external quality control(EQC)schemes,are of utmost importance to maintain the good predictive value of sperm morphology.

    Maternal鄄fetal interface is the position that mother makes direct contact with her fetus.
    There are complex crosstalks between the mother and the fetus. The complicated network among cells,
    cytokines and extra鄄cellular matrix forms a specific state of maternal鄄fetal immune tolerance, which maintains
    normal pregnancy. As a specific subset of T cells, TCR酌啄T cell is an important kind of immune cells residing
    in mucosal and epithelial tissues. TCR酌啄T cells play a role in mucosal immunity. TCR酌啄T cells at maternal鄄
    fetal interface have important implications for maintaining maternal鄄fetal immune tolerance. Hence, TCR酌啄T
    cells are highly relevant with normal pregnancy and recurrent spontaneous abortion(RSA).

    Objective:To search the risk of reduced folate carrier gene(RFC1 A80G) polymorphism of parent and offspring with neural tube defects(NTDs). The purpose is to provide the epidemiological evidence for finding genetic marker of NTDs. Methods: RFC1(A80G) genotype were detected using RFLP-PCR for blood DNA of the 104 triads with NTDs-affected child, and the 100 control families without child-affected birth defects. We investigated the association between the risk of NTDs and parental and offspring’s RFC1 genotype through a case?-parental control study and Family-based association test(FBAT). Results: The parental heterozygosis GA/GA accounted for 36.36%; Twenty?-five percent of probability of offspring’s becoming homozygote GG which was associated with NTDs(P<0.05), indicating that mother and father transmitted G allele to offspring by the probability. The G allele frequency of offspring with NTDs was higher than that of controls when compared to A allele(OR=1.56, 95% CI: 1.07-2.28). There was evidence of association between G allele and the risk of parent having a child with NTDs in the case?-parental control study. However, there was no association between G allele and the risk of NTDs in the FBAT. Conclusions: Our findings indicated that there was no significant association between the risk of NTDs and offspring G allele in FBAT although there was potential association in case?-parental control study. However, the sample size of this study was limited, a larger samples of population?-based study is required to pursue the initial observation.

    Effects of Hydrosapinx on Endometrial Receptivity

    IVF/ET is a better alternative to those patients with severe distal tubal disease. However, the presence of hydrosalpinx has a negative effect on IVF/ET by decreasing the pregnancy rates and implantation rates compared with patients undergoing IVF/ET for tubal disease without hydrosalpinx. The hydrosalpingeal fluid has been demonstrated to be embryotoxic to developing embryos. Poor endometrial receptivity has also been demonstrated in the presence of hydrosalpinges. This article reviewed the latest progress of hydrosalpinx on endometrial receptivity from two aspects: ultrasonic parameters and molecular biological parameters .

    Uterine arteriovenous fistula (UAVF) belongs to uterine vascular disease, which refers to the abnormal direct traffic between uterine artery branches and vein branches by passing the capillary network. The acquired UAVF is often secondary to uterine trauma. Due to the certain similarity in the B-ultrasound manifestations between pregnancy residue after abortion and UAVF, the accuracy of diagnosis may be interfered. A case of mixed mass of left posterior wall of uterus with extremely rich blood flow signals and suspected UAVF was reported. After oral short-acting contraceptive, hysteroscopic surgery was performed, and the pregnancy residue was diagnosed. Combined with literature review, this case report provided us more experience for the differential diagnosis, treatment and prevention of pregnancy residue after abortion and UAVF.

    Relationship Between Pregnancy-associated Plasma Protein A and Hepatitis B Virus Infection and Missed Abortion

    Objectives:To explore the impacts of PAPP鄄A (Pregnancy鄄associated plasma protein A)
    and HBV (Hepatitis B virus) on missed abortion. Methods: The serum and tissues of villus and decidua of the
    missed abortion patients (experimental group) and the normal pregnancies that have artificial abortion(control
    group) were collected. The serum level of PAPP鄄A was detected by double antibody Sandwich enzyme鄄linked
    immunosorbent technology (Sandwich ELISA); The HBV鄄DNA level in tissues of villus and decidua was tested
    by using the quantitative polymerase chain reaction (PCR), and the relationship between the level of PAPP鄄A
    and HBV鄄DNA load was analyzed. Results: The levels of PAPP鄄A of the experimental group (whether or not
    infected with Hepatitis B) were all lower than that of the control group. For the subjects with HBsAg positive,
    the DNA seropositive rate of the experimental group (74.36%) was higher than that of the control group (15%,
    P<0.01). For the subjects with HBV鄄DNA positive, the serum DNA load and the DNA load in the tissue of
    villus and decidua being higher than 1.0伊102 of the experimental group was higher than that of the control
    group (P<0.01, P<0.05). HBV鄄DNA load in villus tissue showed positive correlation to the virus load in the
    peripheral blood (P<0.05). Conclusions:The decreasing of serum PAPP鄄A level in early trimester of
    pregnancy can predict the probability of missed abortion. The higher serum HBV鄄DNA load of the pregnancy
    women was one of the factors that caused intrauterine embryo HBV鄄infection in early pregnancy and the missed
    abortion.
