输卵管妊娠早期诊断和治疗方法的进展及效果评价

摘要： 输卵管妊娠是最常见的异位妊娠，近年来国内外报告发病率呈上升趋势，但输卵管妊娠破裂导致的死亡率呈下降趋势，这主要是因为对其进行了早期的诊断和治疗。阴道超声联合血清人绒毛膜促性腺激素（hCG）和孕酮水平可早期诊断输卵管妊娠。而对不能明确妊娠位置的异位妊娠的预测及处理可提高治疗效果，改善治疗结局。血清β-hCG、孕酮和CA125水平对预测输卵管妊娠结局有一定意义，对选择恰当有效的治疗方法有帮助。输卵管妊娠的早期治疗以腹腔镜手术和药物治疗为主，在掌握适应证的前提下，这两种保守治疗能够取得良好的疗效和保护生育功能。就近年来输卵管妊娠的早期诊断、治疗方法及相关效果进行综述。

Abstract: Tubal pregnancy，the most common ectopic pregnancy, has a increased incidence in recent years. However, the mortality trends to decrease mainly due to early diagnosis and treatment before rupture. Vaginal ultrasound examination combined with serum levels of hCG and progesterone can help early diagnosis of tubal pregnancy. The treatment outcomes can be improved by timely prediction of ectopic pregnancy for those patients with indistinct position of embryonic implantation. Serum levels of β-hCG，progesterone and CA125 are useful to predict the outcomes of tubal pregnancy and helpful to select a suitable therapy. Laparoscopic surgery and medical treatment are main methods for treatment of ectopic pregnancy. According to indications，these two kinds of conservative methods could obtain good effect and fertility preservation. In this article，the early diagnosis and treatment of tubal pregnancy were reviewed.
