Fertility Preservation in Young Female Cancer Patients

摘要: 放化疗技术的进步,使年轻女性癌症患者的生存率得到明显提高。放化疗的不良反应之一是损伤卵巢功能致生育力下降,影响患者治愈后的生育能力。如何保存年轻癌症患者的生育力,日益受到重视。辅助生殖技术(ART)和妇科手术的应用,为癌症患者保存并恢复生育力提供了有效途径,可根据肿瘤的性质和范围、患者年龄来制定相应的治疗原则和个体化方案。乳腺癌、淋巴瘤及卵巢肿瘤等多种患者在治疗后获得生育力已经取得成功。加强癌症专科和生殖专科之间的医疗协作,向癌症患者提供保存生育力知识的医疗咨询,为癌症患者提供保存生育力的治疗,将纳入癌症诊疗计划的重要内容。

Abstract: The improvement of radiotherapy and chemotherapy results in the increases in young cancer survival rates. Unfortunately, many of the treatments are toxic to the ovarian function, finite supply of oocytes and diminish a woman’s chance of future childbearing. Young female cancer survivors have put added emphasis on the need for fertility preservation services. Assisted reproductive techniques(ART) and fertility-sparing surgery are currently available options to increase the likelihood of childbearing after cancer treatment. The currently available options for fertility preservation should be discussed with every patient who is in their reproductive years prior to initiating any treatments. Fertility preservation strategies and protocols should be discussed and performed individually to every cancer patient. As a result, survivors of breast cancer, lymphoma and some ovarian tumors could obtain their child currently. Reproductive clinicians should work closely with oncologists to counsel patients about the treatment on their future fertility and discuss options to preserve potential fertility before starting treatments. Greater emphasis should therefore be placed on fertility preservation during the cancer treatment planning process.
